NEW STUDENT ENROLLMENT
New student enrollment will be Thursday, Aug. 5, from 9 a.m. until 6 p.m. in the main cafeteria. Students Pre-K through 12th grade will be enrolled at this time.
The Caring Van will be available from 10 a.m. until 2: p.m. to administer free immunizations.
To qualify for Pre-K students must be four on or before Sept. 1, 2010. Oologah-Talala Public Schools offers A.M. Pre-K classes which begin at 8 a.m. thru 10:30 a.m. The PM Pre-K class times are 12:10 p.m. until 2:40 p.m. Transportation is not provided for Pre-Ker's.
Students enrolling for grades sixth through 12th must present a copy of their transcript upon enrolling. New students grade sixth through 12th will not begin classes until their transcript is received. A student's enrollment will not be complete until all documents have been submitted.
Documentation required for enrollment:
Proof of residency
Acceptable proof includes one of the following: current, original, full page electric, gas or water bill in the name of the care provider reflecting the service address. CURRENT mortgage documents, a CURRENT lease agreement, or a Real estate purchase contract (NO CONTINGENCY CLAUSE) reflecting the property address, agent's name, address and phone number.
If you use a Post Office box as your mailing address, we must have your 911 address also.
An original state certified birth certificate. No hospital certificates.
Updated Immunization records from the health department of doctor's office must be presented for students to enroll. Immunizations must be current and up to date.
Proof of Guardianship (if applicable). If the enrollee is not living with both birth parents you must provide proof of guardianship (court document or divorce decree).
Parent/Guardian's drivers license.
Other documents (if applicable): Certified Degree of Indian Blood (CDIB) Card, Special Education Records (IEP) and Gifted/Talented Records.

SENIOR PORTRAITS
Oologah High School senior portraits will be taken Wednesday, August 4 from 8 a.m. to 2 p.m. in the school auditorium. There is a $15 sitting fee for Ruth Kelly Studios and various backgrounds will be available. All seniors must follow the school dress code.
ATHLETE PHYSICALS
O-T athletes will have another opportunity to take physicals before the start of the school year. Physicals will be given Friday, August 6 in the lower elementary gym starting at 8 a.m. The cost of the physicals is $15 and forms are available at the football office.

"BACK-TO-SCHOOL" IMMUNIZATION CLINICS SCHEDULED
Back-to-school immunization clinics are in full swing at the Rogers County Health Department. Parents and caregivers should check their children's immunization records to be certain they are current for the upcoming school year. If not, now is the time to schedule an appointment with your health care provider or visit the Rogers County Health Department.
The following back-to-school immunization clinics have been scheduled by the Rogers County Health Department:

Walk-In Clinic
Tuesday, July 27, 4 - 6:3 p.m.
Thursday, July 29, 4 - 6:30 p.m.
Friday, July 30, 10 a.m. - 2 p.m.
Children under the age of 18 years must be accompanied by a Parent or an Adult with the child's Parent/Guardian's permission given in a letter.
Please bring a copy of the child's immunization record(s).
"Vaccinating our children against preventable diseases is one of the most important ways parents and caregivers can protect their children," said Rogers County Health Department Administrative Director Mary Beth Murray. "When we don't vaccinate, we leave the door open for the return for diseases such as measles, which continues to be a threat to children in the United States. School vaccination laws have helped eliminate major vaccine-preventable diseases. Avoid the rush and take the time now to schedule your children for their back-to-school vaccinations."
The following immunizations are required for children entering childcare or school.
Children entering childcare should be up-to-date for their age with:
* Hepatitis A vaccine, with the first dose due at 12 months of age and the second dose due six to 18 months later
* Three doses of hepatitis B vaccine by 19 months of age
* One dose of varicella (chickenpox) vaccine due at 12 to 15 months of age or a statement from the parent or doctor confirming the child had chickenpox disease
* One dose of MMR (measles, mumps, rubella vaccine) due at 12 to 15 months of age
* Four doses of DTaP (diphtheria, tetanus and pertussis vaccine) at 2, 4, 6, and 12 to 18 months of age
* Three doses of IPV (polio vaccine) at 2, 4, and 6 to 18 months of age
* Two to three doses of Hib (Haemophilus influenzae type b vaccine) at 2, 4, and 6 months of age or 2 and 4 months of age depending on the type of Hib vaccine used. Children usually receive three to four doses of Hib vaccine, but due to a nationwide shortage of Hib vaccine, some children will not receive the third or fourth doses.
* One to four doses of PCV (pneumococcal conjugate vaccine) at 2, 4, 6 and 12 to 15 months of age
Children entering pre-school must be up-to-date for all of the vaccines required for childcare, except Hib and PCV.
All children entering kindergarten through the twelfth grade in Oklahoma are required to have:
* Two doses of MMR vaccine
* Two doses of hepatitis A vaccine
* Two or three doses of hepatitis B vaccine (Adolescents 11 through 15 years of age can receive a two-dose series)
* Five doses of DTaP/DTP
* Four doses of polio
* One dose of varicella vaccine or a statement from the parent or doctor confirming the child had chickenpox disease.
Even though the following vaccines are not required to attend school, they are recommended for children ages 11 years and older:
* A second dose of varicella vaccine
* A Tdap booster to protect against whooping cough, tetanus and diphtheria, because immunity decreases with time
* Meningococcal conjugate vaccine (MCV4) because the risk for this disease increases from 15 through 24 years of age
* Human papillomavirus (HPV) vaccine for girls to prevent two types of the virus that cause 70 percent of cervical cancer.
College students in Oklahoma are also required to present vaccination records. All college students are required to have MMR and hepatitis B vaccines and first-time college enrollees who will live on campus are also required to receive the meningococcal vaccine (MCV). The college requirements do not apply to students enrolling only in courses delivered via the Internet or through distance learning in which the student is not required to attend class on campus.
